Please review the following checklist below to ensure you meet the scholarship requirements.

1. Scholarship Application (completed and signed)

2. Must be enrolled in an accredited nursing program such as a Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN) program.

3. Applicant must be in their second semester of the nursing program and maintain a 3.5 gpa. Current Transcript is required.

4. Essay detailing applicant’s career goals and how he or she believes their course of study will further those goals. Please state why you feel you are worthy of receiving the scholarship.

5. Letter of recommendation from advisor, professors, instructors or healthcare professionals who can attest to your skills and potential as a nurse.
